Healthy Communities Delaware (HCD) is a collaborative initiative that works on community-driven priorities around the vital conditions—conditions into which we are born, grow, live, learn, work and play that affect our health. HCD builds capacity for local-level organizations and coalitions through providing funding and technical assistance, and uses IP3 ASSESS to explore and share data for local communities working to advance equitable well-being throughout the state of Delaware.
Healthy Communities Delaware (HCD) is a collaborative initiative that works in partnership with Delaware communities experiencing inequities. HCD uses a collaborative, place-based approach with the goal of collective impact, and emphasizes inclusive, community-led strategies to ensure that community members inform and share in ownership of the work.
Challenge
Among other initiatives, Healthy Communities Delaware (HCD) leads organizations throughout the state who are working to improve community conditions and are prioritizing community-driven work. HCD required a simple solution to explore data organized by the vital conditions framework, as well as highlight data around specific vital conditions and other topic areas and priority populations to equip partners to understand their community and identify where to invest to achieve the best results.
Solution
Through partnership with IP3 and access to IP3 ASSESS, the HCD team can explore and share data with communities, organizations, and collaboratives around the state, with a focus on identifying and minimizing disparities. The platform’s shareable reports include compelling data visualizations, enable users to spotlight specific topics or areas of need, and are integrated seamlessly with resources on Community Commons, including policy guides, toolkits, and more to inspire data-driven action. Over the past year, the Center has also worked with IP3 to add custom data to the platform—namely, data that are specific to West Virginia and aid in understanding the vital conditions. Having familiar datasets from trusted state sources has been beneficial in helping the Center to gain buy-in and prompt collective action.
IP3 ASSESS enables HCD and their group of engaged stakeholders and coalitions to easily explore, share, and interpret data for every census tract and county in Delaware to see how data vary across the state to prioritize areas for investment. The platform allows their team and their partners to view data organized by multiple frameworks, giving the ability to look not just at health outcomes, but at the root-cause community vital Conditions.
